MIAMI/DADE Comprehensive Care Management (CCM) Program

Program Overview

In 2009, the Florida Legislature made changes to the law impacting the Medicaid Program and the delivery of Home Health Services. This website provides information on changes occurring as part of the Comprehensive Care Management (CCM) Program.

Effective July 1, 2010, the Agency for Health Care Administration (AHCA) contract with KePRO to implement a project to monitor on-site delivery of Home Health Services in Miami-Dade County. KePRO shall implement a comprehensive care monitoring (CCM) program for providers of Home Health Services, which includes face-to-face assessments of the recipient by a licensed registered nurse, consultation with the physicians ordering the services, and on-site or desk reviews of recipients’ medical records. The staff hired to effectively perform the different stages of the program are seasoned licensed health care professionals with experience in the Home Health Service field.

The objectives of the program are to reduce and prevent unnecessary program cost to the State, identify fraudulent or wasteful use of Medicaid Home Health services, and in the process, educate all stake holders about the goals and objectives of the program, along with the ramifications of fraud and abuse.
